Patient Information and Forms

WELCOME TO THE OFFICE OF DR. KALLEN K. HULL!

In an effort to reduce paper forms and to keep contact to a minimum, we are transitioning all of our patients to our online forms platform. The process takes just a few minutes and can be done from a PC, mobile device or tablet (please note that Internet Explorer is not a supported browser).  

IF THIS IS YOUR FIRST VISIT TO OUR PATIENT PORTAL

Please be sure to complete the forms in their entirety.  As you move along from page to page, you will need to click the "Save" or "Submit" button at the bottom of each page (different pages may utilize "save" or "submit").  At any time, you may log out of the portal by clicking the "log out" button, which can be found at the bottom of the forms menu on the left hand side of your screen.  On a PC, this menu typically defaults to being open.  On a mobile device you may need to tap the menu button to display the menu, then log off. 

IF YOU ARE RETURNING TO THE PATIENT PORTAL

At times, we may ask you to update your health information which will require you to return to our online portal. When asked if you have been to the portal before, click "yes" and answer the questions when prompted to do so.  Once you provide your answers, a link will be e-mailed to the address you listed.  All information, including the e-mail address that you initially provided when you filled out your forms for the first time must match the information you provide when you return.  

Once you regain access to the portal, you will notice that all of the information you originally provided will still be there.  You do not need to fill out all of the forms again - simply change whatever information needs to be changed and be sure to hit the "save" or "submit" button and then log off.

COVID QUESTIONNAIRES

If you are returning to the portal to fill out a COVID questionnaire to prepare for an upcoming visit with us, you need only fill out that form. You will access the portal in the same way as described in the "IF YOU ARE RETURNING TO THE PATIENT PORTAL ABOVE" instructions. 

 Kindly note that at this time we are requiring a COVID questionnaire for each visit.  In order to have the most up-to-date responses, we request that you fill out the questionnaire within 24 hours of your visit.  The COVID form is the first form on the menu, therefore, when you are done filling out your form, simply click "save" at the bottom of the page and then log off.

ABOUT DENTAL BENEFITS

To get the most up-to-date list of dental insurances we work with, it would best to contact us directly. 

As a courtesy, our staff will contact your insurance company to find out what your benefits are, however, we encourage our patients to educate themselves about their specific dental insurance plans. It is important that our patients have a good understanding of their policy's benefits and limitations.

Most dental insurance plans do offer some benefits that may be covered at 100%, such as cleanings. Certain procedures, however, are only paid at a certain percentage or may have frequency limitations, and will require a copayment on the part of the patient.
 
We are happy to submit claims and/or pre-treatment estimates to your dental insurance. Copayments, deductibles, and any procedures that are unpaid by your insurance due to plan limitations or lack of benefits at the time the claim is processed are ultimately your financial responsibility, therefore it is important to have a thorough understanding of your benefits.
