GENERAL INFORMATION
PATIENT PORTAL INSTRUCTIONS
In an effort to reduce paper forms and to keep contact to a minimum, all of our forms are now done online through our patient portal. The process takes just a few minutes and can be done from a PC, mobile device or tablet (please note that Internet Explorer is not a supported browser).
Prior to each visit to our office, we will send a link to you which will allow you to visit (or re-visit) your patient portal. Please note that the patient portal may only be accessed via direct link that is generated by our office.
IF THIS IS YOUR FIRST VISIT TO OUR PATIENT PORTAL, a link will be sent to you directly from our office to setup a username and password for our portal. When prompted to answer if you have visited the portal before, tap or click “NO,” and then follow the prompts to begin filling out your paperwork. IF YOU ARE RETURNING TO THE PATIENT PORTAL, tap or click “yes” when asked if you have visited the portal before, then follow the prompts to update your paperwork.
Before each scheduled visit with us, we will send you a link to update your health information, which will require you to return to our online portal. The name, date of birth and e-mail address that you initially provided when you filled out your forms for the first time must match the information you provide when you return.
FOR PATIENTS WITH DENTAL COVERAGE
If you have dental insurance benefits, as a courtesy, our staff will contact your dental insurance company to find out what your benefits are. Copayments, deductibles, and any procedures that are unpaid by your insurance due to plan limitations or lack of benefits at the time the claim is processed are ultimately the patient’s financial responsibility, therefore we encourage our patients to have a thorough understanding of their benefits. A common misconception about having dental benefits is that it will cover all treatment, however, this is not the case. Your dental benefits are dictated by the contract your employer negotiated with your dental insurance company. Even in cases where a pre-authorization for treatment was issued, the dental insurance company reserves the right to deny payment when they receive our claim for payment.
No insurance? No problem! We have an in-office membership plan that can help individuals and also business owners looking to provide coverage for their employees. To learn more, please visit our Kleer Membership tab. You may also click on the "Looking for a better dental plan?" section in the lower left-hand corner and then click on "I'm a patient." Employers can click on "I'm an employer."
APPOINTMENT POLICY
As a courtesy, we send electronic reminders and/or place reminder calls within one week of your appointment. If you are unable to keep your appointment, please contact us at least 48 hours in advance. Similarly, if it is evident that you will be more than 15 minutes late for your appointment, please call so we can determine if your appointment should be rescheduled. We realize that extenuating circumstances may delay your travel time, but we need to consider what is in the best interest for you and also for the patient scheduled immediately after you. Kindly note, cancellations under 48 hours and failing to show up for scheduled appointments will incur a charge.
